Care Management
Our care management program is coordinated by a geriatric care social worker that can assess, oversee and advocate for our patients care. Coordinating care is one process we use to communicate our patient’s care, condition and progress.
Disease Management
Improving quality of care, Care Management, Health Service, Monitoring, Registered Dietician and Communication are services we provide that help prevent acute care episodes.
Elite Services
Our Elite Service is provided at independent living and other selected facilities. It is designed to help patients remain independent longer and enhance the quality of life. It is a short stay service as well as an “A La Carte” service to help with activities of daily living.
Skilled Nursing
We teach and instruct patients and/or family infusion techniques and how to maintain an Intravenous (IV) line. Our nurses will draw laboratory specimens as ordered by physicians, when scheduled or as needed. Provide wound care services including, wound vacuum assisted closure (VAC) therapy and versatile 1 Wound Vacuum System. Nurses assess, monitor and administer medication as well as direct treatment according to physicians’ orders. Our staff develops a plan of treatment and goals with close involvement of patients and/or family to achieve self-care. A patient’s care is managed and evaluated to prevent potential complications, promote recovery and ensure safety.
Physical Therapy
Provides evaluation, including: muscle strength, joint motion tests, sensory coordination tests, balance tests, observation, palpation flexibility tests, postural screening and movement analysis). Our staff develops a treatment plan for dysfunction problems; generally a Physical Therapist (PT) assists with gross motor lower extremity activity.
Occupational Therapy
An Occupational Therapist (OT) provides activities and instruction designed to increase independence for the patient who is unable to perform activities of daily living (e.g. sensory impairment, perceptual motor difficulty, coordination problems, muscle weakness and energy conservation). Generally, an OT assists with fine motor, upper extremity level.
Speech Therapy
A Speech Therapist provides services designed to help patients improve or restore communication skills. One example might be voice and language disorders, such as stutter fluency and rhythm problem, inappropriate pitch, or harsh voice and speech quality problem. Our therapists can address speech disorders resulting from hearing loss, stroke, cerebral palsy, mental disability or brain injury.
Medical Social Worker
Our Social Workers provide social, environmental and emotional counseling on relationship problems, coping mechanism, long term illness and living arrangement issues.
Home Health Aide
Our aides works closely with patients to provide assistance with daily tasks, such as dressing, bathing, feeding and bladder/bowel function assistance. The aide will also check vital signs, oral hygiene and be responsible for ambulation, repositioning and household functions.
Personal Care
We are also a provider of personal care services including caregiving & companionship services. Our live-in and hourly services & care include:
